Hi North,
My thoughts about what causes the jets relate to a spinning black hole in an ether medium. The spin causes an increase in gravitational force in the space around the equator of the black hole (this is similar I believe to what causes the dark matter gravitational effect).
At the poles there is no increased gravitational effect and some matter that gets sucked in finds an escape route via the poles.
If the black hole is very active there is more chance that some matter will be force out from the poles.
I believe that black holes have a surface and are not points of singularity and that the jets forms on the poles at their surfaces.

hoping for another explaination, something other than....but two things, remember there are jets at both poles and if some matter escapes through the poles what gives it the ordered "jet" configuration?
